USEPA has revised the Hazardous Waste Manifest Forms used to track hazardous waste from a generator’s site to the site of its disposition. Handlers of waste will obtain new forms from any source that has registered with USEPA to print and distribute the form. Illinois is one provider of the revised manifest forms.
USEPA maintains a Manifest Registry that provides assistance to the public in obtaining the new manifests. Please note that the revised Uniform Hazardous Waste Manifest was required as of September 5, 2006.
The information below explains the State of Illinois’ manifest requirements.
Pursuant to Public Act 99-55, effective July 16, 2015 generators do not have to submit copies of hazardous waste manifests to the Illinois EPA. This supersedes other rules adopted by the Illinois Pollution Control Board regarding manifests at 35 Ill. Adm. Code Part 722.
You may order manifests from the State of Illinois by mail with a check or money order.
Illinois requires non-hazardous special waste to be manifested on the Uniform Hazardous Waste Manifest. Illinois generators and out-of-state generators sending nonhazardous waste to an Illinois receiving facility must use a manifest for all shipments of nonhazardous special waste. All nonhazardous special waste should be identified as either Class A or Class B. Copies of these manifests are not to be submitted to the State of Illinois; instead Illinois receiving facilities are required to submit summary information in the form of the Nonhazardous Special Waste Annual Report by February 1 of each year for the preceding calendar year (the IEPA generator number must be included on that report).
